1.2 Language and wording of the questionnaire:

1.3 Types and forms of questions:

- Open-ended (OE) or close-ended (CE):

OE questions will allow the respondents to express their answers in whatever manner they choose. Catch-all question; e.g. How do you feel? (full text answer required).

CE questions will force respondents to choose between options. Simpler process as common vocabulary. Options offered need to be mutually exclusive and exhaustive; e.g. Do you feel better, worst, same? (3 options to choose from).

- Positively (PQ) and negatively worded questions (NQ): Often used in alternation to ensure respondents pay attention to the questions. Avoids repetitive patterns.

- Double-barreled questions: Avoid questions that include 2 or more points to be answered. Is confusing.

- Ambiguous questions: Ensure that the questions do not confuse the respondents.

- Recall-dependent questions: If a respondent is asked to remember the past, this might generate a low reliability answer.

- Leading questions: Avoid wording that leads the respon-dents to favour one of the options offered; e.g. Do you agree that productivity should be the only base for salary increase? (Yes or No).

- Loaded questions: When tackling a sensitive topic (i.e. sexuality, honesty) be careful with the words used, as emotions or values might vary between respondents. Biases!!!!

- Social desirability: Social standards and values are often imposed on respondents, therefore the researcher will not obtain the truth; e.g. Do you wash your hands after going to the bathroom?

- Length of questions: Maximum of 20 words.

1.4 Sequencing of the questions in questionnaires:

- From the general to the specific

- From the easiest to answer to the ones that need focusing, thinking or reflection

- Might consider mixing questions on a related topic in order to unlink the answers in an effort to get to the truth or validate consistency in the answers.

1.5 Classification data or personal information:

- Often defined as characterizing questions that allow the researcher to establish the demographic information of each respondent

- Locating at the beginning or the end of the questionnaire is a matter of individual choice by the researcher

- When pool of respondents is small, do not ask for too many details in order to retain confidentiality (e.g. only 1 female in a group of respondent: dont ask the gender).

2.1 Appearance of Questionnaire:

- Well organized

- Visually pleasing to the respondents, with professional look

- Font type and size that are easy to read; no need for creativity that might create confusion

- A short thank you note is always welcomed.

2.2 Length of Questionnaire:

- Often difficult to achieve balance between getting all the information the researcher wants and maintaining the goodwill of the respondent (i.e. willingness to take the time). If too long, might need to divide questionnaire in 2 parts.

- It is better to use a variety of types and forms of questions as this will retain the attention of the respondents

- The flow of questions has to be progressive, without generating a bias when moving from question to question.

2.3 Introduction to Respondents:

- Identity of the researcher and description of the purpose

- Establish a rapport with respondent aiming to motivate them

- Reassure the confidentiality for the respondent, reducing biases

- Thanking the respondent for their time

2.4 Instructions for Completion:

- Answering the questions should be intuitive, but just in case, appropriate comments need to be provided

- If divided in parts (i.e. About yourself, Purchasing patterns, etc.) provide appropriate comments if required

- Explain for each question form what is expected from the respondent; e.g. Choose the best answer, allocate a number for each option that sums to 100, etc.
