Using PHP

I have an insert page to to take in data from a user called insert.php$.$ Picture will be below.

I need a page called do$\_$insert.php that will take data from the insert, validate the form data, and then insert that data into a simple database. The database is just a simple one called items with data entry being "Brand", "Product" and "Price".
