Licensing staff have found the licensee and manager to be cooperative and receptive to Licensing. Reportable incidents are reported in a timely manner and staff responses to incidents are generally appropriate. The last routine inspection occurred 3 months ago and a low hazard rating was assessed. The facility was also assessed as low risk, according the assessment completed at that time. Intake: Licensing received a fax of a reportable incident from Sherwood Forest. The date of the incident was the day before the date of the fax. \"Physical abuse\" is checked off, and the description of the incident states: Care aide reported to Director of Care (DOC) Mrs. Whiting that a person in care (PIC) Mr. Smith was physically abused by registered nurse (RN) Ms. Fish on the previous evening shift, while staff were attempting to bathe Mr. Smith. Upon contacting the Mrs. Whiting, she stated that she was contacted by a care aide (CA1) Staff Ashley today and informed that yesterday a registered nurse (RN) Ms. Fish had twisted the arm, and kicked Mr. Smith while Staff Ashley, Ms. Fish, and another care aide (CA2) - Staff Jasmine were trying to get Mr. Smith to have a bath.
