As a health care professional, how can health care organizations implement a better raise policy, so that employees understand how to earn, achieve or accrue a potential raise?

1) How would you determine who is doing good quality work from great quality work, what are the factors? and 2) How would you address those staff members who are doing less than average work, or should average workers even receive that raise at all? Especially if quality is in the mission statement, even average quality should be considered poor quality, thoughts?
