When we learn from what has happened on projects, we should document these lessons learned, and make a plan on how to improve on the stuff that did not work, and intentionally put into action the things that did work. In agile we do this at the end of each iteration. This is called a $\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_\_$ on agile projects.
