Which quality approaches , methods and tools would you propose to mitigate each of the 8 contributing factors identified in the release ? Every operation and processes are independent and critical on the other processes in the organizations , safety and the quality ventures should be the part . To use these safely and quality improvement ventures NASA is such kind of organization. A mitigation plan should be run to avoid those kinds of failure and the all failure scenarios should understand properly The following approaches is my suggestion for the loss of Mars Climate Orbiter . First of all, Execution by DMAIC and project should be analyzed . DMAIC is a component with six sigma approaches and five phases used like measure , define , analyze , control and improve . In the beginning step , by reviewing previous data and by defined concept of expectation a project defined to start . Then all quality approaches can be selected , the aim of performance is defined . NASA , very beginning they used DMAIC in the Mars Climate Orbiter project to run . After failure , myself will suggest using failure modes and effect analysis with root cause In the case of Mars Climate Orbiter , the problem caused by the engineers who failed to convert the English metric units of rocket thrusts to Newtons that is the metric system used for measuring rocket force (Boyle , 2016 ). Root cause analysis (RCA ) is a technique which is used to describe the trends and risk assessment in the case of suspecting of errors caused by human . This is a critical technique to gather the information on causes (Hughes , 2008 ). Underlying errors of reasons can be finding out by using RCA .
