WHAT IS THE PURPOSE OF YOUR SURVEY (2 points) – HOW WILL YOU USE THE INFORMATION Design your questionnaire using: Part 1: 5 closed-ended (or scaled) questions (1 point each) Part 2: 5 open-ended questions (1 point each). Be sure to design the questions very carefully, and to use appropriate and simple words. Do not mix the closed-ended and the open-ended questions.
