b$)$ Outline of the tasks that you plan on doing before arriving on site $($such as review

of relevant documentation, etc.$)$

c$)$ Plan of activities that will be done while on site and the necessary equipment that

you will bring

d$)$ List of non$-$destructive tests that you will conduct while on site $($minimum $3)$ and

rationale for your choices

e$)$ List of destructive$/$laboratory tests $($minimum $2)$ that you are planning to conduct

and rationale for your choices

$3.$ Repair Plan: Assume one of the causes $($or more if applicable$)$ you identified in Part $1$ is The actual cause of deterioration and prepare a repair plan. The repair plan must

include, at minimum, the following:

a$)$ Repair materials

b$)$ Repair methods

c$)$ Surface preparation requirements $/$ concrete removal methods

d$)$ Quality assurance testing to evaluate the properties of the repair materials

e$)$ Estimated quantities of repair

f$)$ Any protection systems that may appropriate
