Write an Initial PSYCH VISIT, chief complaint, , Psychiatric visit HPI, current Psych meds, exam, social history,, screening tools and results MSE, diagnosis and recommended Plan (medication and dosages), rationale for using the medications for 80 years years old female referred to be seen for initial psychiatric evaluation . she is currently at a skilled nursing facility post hospitalization of displaced transverse fracture of left patella. She has psych diagnosis of Epilepsy unspecified, MDD and delirium due to known physiological condition. She is currently on Paxil 20mg daily for MDD, QUETIAPINE FUMERTAE 25MG FOR MDD, And Gabapentin 800mg TID for chronic pain. Assessment was done in the patient's room. She said she was ok, sleeps well and is adjusting well to the facility. She said she is just waiting for the x-rayof left patella to be done. She lives with her husband at hom, has 2 children , 5grandchildren and 11 great grandchildren. She really loves her big family
