A subscriber has a unique subscriber's number, name, date of birth, sex, address, telephone number, insurance plan and the employee that handles the subscriber's paper work. Some queries access the records by the last name of the subscriber. Some queries need to refer to the zip code in the address. e. Each claim has a unique claim number, a subscriber, a date, a type of claim, the local office that accepts the claim, and the employee who processes the claim. f. Each claim may have several follow-up records. The record has a sequence number that is unique within the claim, the date of the record and the nature of the record.
