You will be creating a performance appraisal (also known as performance evaluation) form from scratch using Microsoft Word for a job of your choice. The form that you create will become the base of the form that you use for our performance review together at the end of the semester. My expectation is that you will create a document that appears professionally done and is original (ie. from scratch). The following sections must be included in the document:

  • Proper performance evaluation form headings
  • At least six to eight competencies with multiple key indicators identified for each competency specifically relevant to the job you chose
  • A behaviorally-anchored rating scale (BARS) referring to the key indicators of each competency mentioned in the previous step
  • Total score and average score per competency
  • Explanation of what score ranges qualify or disqualify you for/from
  • Development Plan section--> Include area for specific development goals, resources needed, timeline to be reviewed
  • Additional comments box for employee
  • Appeal statement and instructions to file appeal --> "Signing because the document was discussed, not because you agree with its content" etc.
  • Signature lines --> Employee, Reviewing Supervisor, and (at least one more in a higher, relevant position)
  • If you include results required of the job and the potentially related rewards/consequences, too, you could earn extra points.
