You will be creating a Web Form using HTML and CSS as follows:

NOTE: This form is the "paper equivalent." Yours will be a web form. In other words, your design will NOT look like above. You will structure it to look good on a website. You will use appropriate input fields. For example, Transfer Student should be a Radio Button. Advisor field should be a drop-down select.

You should use the Form tag but leave out "action" attribute. You should add a "Submit" button.

Everything should be done in ONE FILE.
