The intent of this assignment is to create your own personal database of contacts (individual and Company).

Start your project by constructing a "Form" in Access with the fields listed below.

Include a combination of individual and business contacts.

Use organizations, businesses, or service provider company contacts.

• Field Name 1: Contact ID (for all individual and business or service provider contacts): This should be a sequential numbering from 1 to 100 (depending on how many individuals in your contact list
• Field Name 2: Last Name of individual contacts (not company contacts)
• Field Name 3: First Name of individuals (not company contacts)
• Field Name 4: Type of Contact (Family, close friends, acquaintances, work colleagues, business contacts) Use the drop-down option that allows you to select from a list
• Field Name 5: Business or Service Provider Organization Name Only company, organization, business, or service provider names
• Field Name 6: Business or Service Provider Type (Insurance Agent, Dentist, Medical services, Vehicle services, Schools, Lawn care service, HVAC service, handyman, etc.)
• Field Name 7: Business Contact Person Name Do not include these names as a separate Contact ID
• Field Name 8: Type of Phone number (Cell, Home, or Business drop-down option) Use the drop-down option that allows you to select from a list
• Field Name 9: Phone number (format (423) 322-4499) Use the phone number formatting for this cell
• Field Name 10: City of Residency (Business & Individual)
• Field Name 11: State of Residency (Business & Individual)
• Field Name 12: Email address (If available)
