I have been an English teacher for over 1 year, where I started teaching in Mexico to local communities in 2018 and started teaching ESL online this year. I currently work for two online platforms teaching formal lessons to children and also conversational lessons to adults.
This role involved the assessment and analysing of patients and information pertinent to their needs and the systems they functioned within as part of facilitating discharge from hospital for service users who met the threshold for adult social care services as per The Care Act (2014). This role required extensive medical report writing of the needs of patients and of their medical diagnosis and the impact this had on their care needs. This involved assessing the care needs of the service users, and heavy use of the Mental Capacity Act (2005) and ensuring that any services put in place or decisions made are in line with this essential piece of legislation. I was then responsible for the formulation of care and the monitoring and review of this in the community, whilst working in a timely fashion within tight time and resource constraints.
As a project manager I oversaw a particular discharge pathway and supervised the other members of the team. This involved a high level of communication with the multi disciplinary team, families and patients. This role also required a high level of writing skills whereby assessments were put before panel, prior to the funding being approved.
Given the wide stretch of people involved in this process, this role involved being able to adapt and manage both positive and negative communication in order to focus on the task at hand.
