Buffo was employed by the Baltimore & Ohio Railroad. Along with a number of other workers, he was removing old brakes from railroad cars and replacing them with new brakes. In the course of the work, rivet heads and scrap from the brakes accumulated on the tracks under the cars. This debris was removed only occasionally when the workers had time. Buffo, while holding an air hammer in both arms, was crawling under a car when his foot slipped on scrap on the ground, causing him to strike and injure his knee.
