1. Develop the system in Java using the Swing library that interfaces to a MySQL database. 2. Each form (excluding the report form) for displaying data from the database must have buttons to add, modify, save, and delete a record of data that is displayed on the form. 3. You must be able to get to the various general medical history categories from each form. Any references in the patient demographics form or other forms to healthcare providers or other persons must be via an ID from the Person table.
