Identify all the relevant role players who should be involved in the monitoring and evaluation of your programme and project. Briefly explain their roles and responsibilities. 4. Discuss what capacity needs to be developed (if any) amongst the respective role players to ensure the effective monitoring and evaluation of the programme and project. Develop a plan of action to develop this capacity. ( 5. Develop the following for your project or programme by applying the CREAM criteria: a. Outcome b. Indicators c. Baselines d. Targets e. Activities f. Inputs 6. Explain how you intend to collect data to measure the outcomes, indicators, baselines, targets, activities and inputs. 7. Explain the structure of the evaluation report that you will develop for your programme or project. 8. Explain how you are going to communicate the evaluation to relevant role players. 9. Explain how the evaluation reports findings, recommendations and conclusions can impact on the M&E system. 10. Draw up a monitoring and evaluation implementation plan for the project.
