Fact Sheet: You will submit a user-friendly, 2-page (front and back) fact sheet that describes the community resource in which you investigated. Your fact sheet should include the following sections: (a) general overview of what the resource is, (b) description of the supports and services/programs provided, (c) description of who this resource is appropriate for (e.g. students with intellectual disability), (d) who is eligible for services and when, (e) what the services cost or how they are funded, (f) description of the referral process and required documents (if needed), and (g) how and when do they become involved in the transition process.
