1. Set up a time to meet with your client that is suitable for them and for your assessor. {You may also meet with the client's carer or a family member if the client is unable to participate.) 2. Refer to your completed plan from Assessment Task 2, and talk to your client about the community services and activities you identified. You should show them related information as much as possible (for example, you can show them websites, brochures, etc). 3. With your clients (or other approved person's) assistance, work out a strategy that can support their social needs. Your assessor will be looking to see that you: ' Talk to your client and acknowledge their social needs Discuss your plan with the client (or their family member, alternative representative or carer if required) Provide information on services, programs and appropriate options available in the community For example, you may show them the websites as listed in your plan. During the discussion, talk about any cultural or religious needs that may impact on the client being able to access services andlor activities. Work with the client (or their family member or carer) to select the best options to meet their needs. Are supportive, encouraging and informative during the consultation. For example, make sure you present all the facts; explain the pros and cons; allow time for decision making; encourage the client to make their own decisions and suggest new ideas, etc. Consider any specialist or referral requirements. Acknowledge and consider feedback from the client during the discussion. For example, thinking about their ideas; clarifying that you have understood their needs or decisions, etc. Explain to the client how you will review and monitor the effectiveness of the strategy. Confirm with the client the next steps. This includes setting expectations to ensure everyone involved has a clear understanding of how the plan will be put into place; confirming program places, timetables, decision making by client etc
