1.3 Find an example of an actual product failure. Why did it occur? What was the root cause of the failure? What were the consequences? Can you put a value (e.g., time and money) on the consequences?

1.4 In some situations, the de!nition of failure may depend on both the performance speci!cations and expectations. Can you think of a past experience where you considered a product to have failed but it may not have been considered a failure according to the product speci!cations? Describe the situation. If you cannot think of a situation, report a hypothetical case.
