The packet must include:

$$ Names, titles, organizational affiliation of each CAB members $($and rationale for why each was selected$)$ taken from actual County organizations, etc. $.$

$$ Purpose of the CAB

$$ Description of the CAB$$s organizational structure including meeting frequency, format, and how decisions will be made.

$$ Responsibilities and role of CAB members and term limits$.$

$$ Statement of ethics of participation including a description of what constitutes a conflict of interest and compensation for participation
