A transition plan also known as knowledge transfer involves educating the new owners of project tasks on the key elements of the project ( Sipes & FAAN, 2019). It is important in that it will provide all stakeholders with information on the project's completion. It gives a strategy for the successful completion to anyone who might be continuing the project or optimizing the project, thus securing a smooth transfer. A change management transition plan is a document that lays out the tasks and activities to efficiently transition the change management program from the implementation phase to the maintenance phase (Ogbe, 2021).

A Transition plan should include all the tools, techniques, and methodologies required for change sustenance, as well as the ongoing activities that need to be performed to ensure the change is sustained (Ogbe, 2021). It is like a checklist or guidebook headlining all the activities that must be completed. All sponsors, stakeholders, and key team members must be a part of this plan, including any new owners who might be taking over the project. Everyone who holds an important role in the project must have a copy and the official document must be stored in a file where it can be managed and accessible as needed.

Moreover, according to Ogbe(2021), during a transition plan, the project manager must meet with sponsors and key stakeholders to discuss what outstanding issues need to be addressed prior to the change management team being dissolved and work out an acceptable timetable for the transition to occur. It is also very important to prioritize employees during this time to help their financial insecurities, most are afraid of losing their jobs due to either new management or other factors.
