According to Gupta (2014), the survey is a data-collection method typically used to gather data from many people at one time; it can be mailed, emailed, or offered online. During last nights class, I was reminded about this specific survey that my employment place created regarding the COVID-19 vaccine. In this particular survey, the department is attempting to figure out how many of the employees are willing to receive the COVID-19 vaccine soon to roll out. Per usual, after a short clip of the actual research and fact-based information presentation by the upper management, a survey question popped up in the end. Although there are several ways of developing a survey question, for instance, sequence questions, open-ended questions, closed-ended questions, and multiple-choice questions, in this case, I would assume that it should have been a two-choice question of YES or NO or even an added DON'T KNOW option. As shown in the image below, only YES, I want to receive the COVID-19 Vaccine is the only option to be selected to end the E-Learning module.

As far as I can see and tell regarding this survey, the results are either employee will exit the survey without answering the question. It will be incomplete, or it will be a forced answer of YES, and it will not correctly depict the number of people who will actually get the vaccine. Like myself, I am still on the fence if I want to be vaccinated; however, to get rid of the survey on my E-Learning queue, I selected the only option. With no other option in this survey, it lacks the information and the desired result that the organization should have to address the issue and make the skeptical employees more at ease. Unless, of course, the organization's desired result is to have a 100% of their employees say YES to the vaccine, then it is a flawed survey overall.
