If a patients name on a request does not match perfectly to a patients name in your facilitys EHR, then as an HIMT professional you must not release information for Maria Lopez in your records for a request of Marie Lopezs information. Another example is making sure the expiration date of the request is present and denying the request if that deadline has passed because then the HIMT professional will be liable for that.In a hybrid record environment, some records are in print form, which later gets scanned into the medical records and others are entered and stored completely electronically. One example of a potential issue when processing ROI requests in these types of facilities could be that the document has not yet been scanned into the medical record therefore the request might be denied in error. Another issue could be that a printed document was scanned and added into the wrong section of the patients medical record also causing an erroneous request denial.
