• Assigned social determinant of health (explain what social determinants are)
• Community that needs improvement in this social determinant (use data to identify the community)
• Present the community (where is it, the demographics, and the key health issue as related to the social determinant of health)

• Present an action plan to address the social determinant and improve the health outcome for the health issue presented.
• The action plan must be clear and directly related to the issue.
• Indicate if it is a program or policy.
• Present a logic model (well-developed)
• How will the action be measured to ensure that it is working?
• Provide measurable indicators (metrics). They will come from your logic model.

• have an introduction and a conclusion,
• use visuals as appropriate but remember to include short bullet points for the key ideas,
• include a reference slide,
• provide in-text citations within the slides, as needed.
