Using your Program Logic Model from Assignment 3, create an outcome measurement plan that includes:
- Indicators for the outcomes determined in your program logic model (Assignment #3). Each outcome should have at least one indicator. If it is not feasible for the program to measure an outcome (i.e., the outcome will occur far into the future), please provide a rationale explaining this.
- The data collection method that will be used to measure the indicator (i.e., observation)
- The frequency that the indicator will be measured (i.e., quarterly)
- The position that will be responsible for measuring the indicator (i.e., the program manager).
The template for the outcome measurement plan will be provided by your instructor.
