Objectives:

Apply knowledge gained from To Err is Human and Crossing the Quality Chasm or from Hospital Compare$/$Leap Frog Group to a specific quality problem in the organization they selected for their project in Week $2.$

Conduct a scan or information$-$gathering exercise of the organization selected in Week $2.$

Analyze various quality issues and select one which is relevant and problematic to the organization.

Describe the significance of the issue, in terms of the risks it could present if not fixed.

Explain how this issue can be related to the quality issues identified in To Err is Human and Crossing the Quality Chasm.

Purpose:

This assignment will give students an opportunity to consider what they have learned about specific quality issues in healthcare, conduct a scan or information$-$gathering exercise into the organization they have selected as a project topic, and identify one of those quality issues that is relevant and problematic in that organization. It will also give students the opportunity to explain how this issue can be related to those quality issues identified in To Err is Human or to the Six Aims identified in Crossing the Quality Chasm. $($This could integrate work submitted in week $2).$ In this regard, it addresses how the issue may not be one that is isolated in their organization but is identifiable in others as well. This assignment should also be used as part of the final deliverable in week $7.$

Directions:

With the organization you selected in week $2$ as the subject to your quality improvement project, select a significant healthcare quality issue within the organization. Write a paper of at least three $(3)$ pages on the issue and how it affects patient care and patient quality.

How does this issue relate to either the errors identified in To Err is Human and$/$or the Six Aims identified in Crossing the Quality Chasm?
