- What is the courtroom number?
- Who is the presiding Judge?
- What is the style of cause?
- Identify the parties.
- Identify the court supporting staff assisting the judge.
- Are the parties self-represented? If not, who are the legal representatives? Paralegal, lawyer, or other representative?
- What is the nature of the proceedings?
- Provide details of the matter. Including relief and amounts sought and types of damages.
- Provide details of any evidence that was submitted or referred to.
- Provide details of any preliminary issues dealt with before the hearing. Ex. Jurisdiction, evidence, service of documents, rules of pleadings, witnesses, limitation periods, exclusion of witnesses, adjournments, etc.)
- Was there a final order? If so, provide details.
- Were there submissions on costs? Any mention of offers to settle? If so, provide details.
- Explain your experience and what you learned.
