Whenever patients are seen for the first time, they complete the registration form that asks their name, address, phone number, and brief medical history. The clerk at the counter received the form and stored it in the patient information file. When a patient call to schedule a new appointment or change an existing appointment, the clerk checks the appointment file for an available time. Once a good time is found for the patient, the appointment is scheduled. If the patient is a new patient, an incomplete entry is made in the patient file such as their name and contact number. The full information will be collected when they arrive for their appointment. Before hang-up the phone, the clerk confirms again with patient about the appointment and updates the appointment book. Because appointments are often made far in advance, the clerk usually mails a reminder postcard to each patient two weeks before their appointment. Every morning the on duty dentist will check the appointment list and the medical history of the patients for that day. After every treatment, the dentist will update the patient file with the treatment given. The most common treatments are extraction, restoration, scaling and root canal. Patient collect the invoice from the cashier and pay the amount either with cash or credit card. For a credit card payment, cashier will check for card authorization first.
