Blezdel Semiconductor is an electronics manufacturing company whose output per quarter for their plastic package of 18 Leads are as follows:

Q1 190,000 pcs, Q2 175,000pcs , Q3 140,000pcs, Q4 115,000 pcs.

Upon analysis done by the quality assurance and the production people, they found out that the decline in output resulted from poor workmanship from the production people as evidenced by 208 units found to be defective due to this. Production people are marking the wrong code( 286 units ) on the ceramic packages. This is due to the fact that many of them have no appropriate training.

There are also plenty of packages with cracks ( 390 units ) as a result of equipment being used for production not been done regular preventive maintenance, thus, equipment is not accurate and breakdown is frequently happening. The QA also found out that 750 pcs are having misaligned lead due to machine malfunction . There are 100 pcs which are rejected due to dust/dirt while 25 pieces are rejected to failure in the hot temp test.

A failure is occurring in the hot test if there is issue with the process or method.

QA also noted that some portion of the raw material itself is substandard ( 100 units ) thats why the end product is rejected. Dirt and stains ( 130 units ) in the products are notable due to poor working environment which is disorganized and dirty.

Some of the rejects are caused by poor quality as a result of not following the standard procedure. As per standard, the length of the leads should be 1.5 cm +/- 0. 25 cm. As of QA data, the average lead measurement per month are as follows:

Jan 1.65, Feb 1.40 . March 1.55, April 1.62, May 1.38, June 1.45, July 1.58 August 1.75 Sept I.52, Oct 1.58, Nov - 1.62, Dec - 1.40.
