Your supervising attorney has asked that you obtain copies of the client's medical records from each of the hospitals and doctors.

Prepare an Authorization for Release of Medical Records allowing your law firm to obtain these records of your client's treatment. Remember, requests for medical records should always be HIPAA compliant. (Hint: the example 4-3 in your book is not HIPAA compliant. If you don't know what HIPAA is, research it first). You may make up the names and addresses of doctors or hospitals.
