While shopping at Walmart, you purchased 4 boxes of Hefty Tall Kitchen Garbage Bags, each containing a $5 rebate for which you were eligible. Upon arrival back home, you completed the rebate forms and mailed them with the required proofs of purchase and receipt; three weeks later, your $20 rebate check arrived. Unfortunately, you got busy and promptly forgot about the rebate check---until 3 months later when you found it while looking for a postage stamp.

Thus, you decide to write a claim letter requesting a replacement check from the rebate center, YA, Inc., whose address is 10 South 5^th Street, Minneapolis, Minnesota, 55402. Of course, you will enclose the original, expired rebate check to help expedite the replacement process. You also investigated and decided to mail your letter directly to the person at YA, Inc., that is in charge of the customer service team: Kathleen V. Cowden.
