Identify Failure Modes: Identify potential failure modes within the selected process or product. A failure mode is defined as any way in which the process or product could fail to meet its intended function.

Assess Severity: For each identified failure mode, assess the severity of its potential consequences. Use a numerical scale (e.g., 1 to 10) to rate the severity, considering factors such as safety implications, customer impact, regulatory compliance, and financial consequences.

Assess Occurrence: Estimate the frequency or likelihood of each failure mode occurring. Again, use a numerical scale (e.g., 1 to 10) to rate the occurrence based on historical data, expert judgment, or other relevant sources.

Prioritize Actions: Based on the RPN scores, prioritize the failure modes for corrective actions. Identify potential mitigation strategies, such as design improvements, process modifications, enhanced monitoring, or redundancy measures.

Document Results: Summarize your FMEA analysis findings in a report format. Include a description of the process or product analyzed, a list of identified failure modes, severity, occurrence, detectability ratings, RPN scores, prioritized actions, and any additional insights or recommendations.
