This is an employee survey to go in a proposal to implement a Telecommuting Pilot Program to allow employees to work from home for a fake call center.

You will outline a Likert Scale survey for the data you need to collect and the data collection process. While developing your outline, keep the following in mind:

  • Will the survey gather the information you need to gather? (relevancy)
  • Does it accurately reflect real-life market and organizational conditions? (validity)

Then you will write 10 survey questions/statements to better understand how others feel about your proposal. While writing your anonymous survey, keep the following in mind:

  • Does the wording of your questions and the design of your survey work together to obtain accurate responses?
  • Are the questions worded in such a way as to avoid leading or loading?
  • Is every question necessary?
  • Are the response choices easy to understand?

Example: If you are trying to determine if other employees feel adequately trained in the different aspects of the job, one statement might be:

Q1: I believe I was effectively trained

  • Strongly Disagree

Conclude your survey with five additional questions that ask the demographics and psychographics of the survey participants. What would the decision-maker want to know? Think in terms of position, management level, years with the company, department, education level, type of transportation to work, work shifts, etc

  • 1-2 years
  • 3-4 years
  • 5-8 years
  • More than 8 years

  • Restate the Topic or Opportunity
  • Objective of the Survey (what you hope to learn - is the topic or opportunity a shared perception or is it just yours?)
  • Intended audience of Survey (employee groups)
  • Attitudinal Survey Questions (10 questions - be sure to screen and evaluate questions with criteria described above)
  • Demographic Survey Questions (5 demographic questions - be sure these are relevant to your targeted participants. For example, if surveying employees, be sure to find out their rank, years, with the company and job satisfaction)
