Create a database for a veterinarian office. The database design needs to conform to the

following requirements:

$1.$ The database needs to track customers $($including contact information and where they

live$).$

$2.$ It needs to track pets $($including type of animal $$ dog, cat, etc, who owns the pet$).$

$3.$ It needs to track visits $$ ie pet visits to the veterinarian.

$4.$ When a pet visits the veterinarian they could have $0$ or more diagnosis. This table will

contain values for all possible diagnoses.

$5.$ Each diagnosis might have $0$ or more treatments. This table will contain records for all

possible treatments.

$6.$ The database must conform to the $3$rd Normal Form.

Your design does not and should not need to include employees of the office, or

appointments.
