Create a php web page (name the file bmi.php) that displays a person's BMI (Body Mass Index) based on the weight and height values that are passed to the web page as data in the form. The formula is:

 BMI = 704.5 * (weight / height2) where weight is in pounds and height is in inches 

Your php script should get two values for the height since that is how people usually identify their height in feet and inches (such as 5 feet and 10 inches tall). The weight given will simply be the number of pounds the person weighs (such as 168.5 pounds). The submit should also pass in the person's name so that you can include the name of the person that this calculation is for in the web page.

You php script should generate all of the following, including the major heading and a sub-heading (with your name and course), the BMI table (as an HTML table), and the person's Name, Height, Weight, and BMI, as suggested below:
