Create a survey using the program of your choice. The survey should have a title, an introduction, and at least 20 questions which must assess employee knowledge of privacy, security, and consent as it applies to interoperability and data integrity. In addition, you will include the link to your survey.

You will provide a written response for each of the 20 questions for employees to refer to for review of compliance requirements.

Provide a brief summary of educating staff on the importance of privacy, security, and consent to ensure data integrity when promoting interoperability. Include challenges associated with data integration and two examples of consequences of inadequate data integration.

Create a three-panel storyboard that illustrates a role-play between two employees demonstrating their knowledge of privacy, security, or consent.

Requirements

• Using the survey creation program of your choice create a 20-question survey.
• Include the link to the created survey.
• Provide a one-paragraph explanation for each of the 20 questions for employees to use as a learning tool.
• Create a 500-word summary providing an overview for staff on privacy, security, and consent.
• You can use a storyboard template of your choice, use Skitch software, or upload digital versions of your panels (three panels minimum, does not include a title panel).
• You should include at least three peer-reviewed sources.
