HPI: Patient is an 80-year-old female with history of depression and ETOH abuse seen today for medication monitoring. Observed with a good mood in the room watching TV.

PMH:

Medical History: Vit D3 deficiency

Psychiatric History:

F418 Other specified anxiety disorders

G308 Other Alzheimer's disease

F1099 Alcohol use, unsp with unspecified alcohol-induced disorder

F331 Major depressive disorder, recurrent, moderate

Medications:

Lexapro 20 mg daily

Melatonin mg HS

Allergies/Intolerance: N.K.A.

Family History: No family history

Social History: No history of alcohol and illicit drugs use. Sleeps well with good appetite

Assessment: Present to be forgetful, confused, with euthymic mood and reactive affect. Polite and corporative. No s/s of hallucination and SI/ SH.

Treatment/ Plan:

No new medication recommendation

Continue all current medications

Follow up in 3 months."
