This assessment will be referring to the incident with Craig that we began to investigate in Day 3 Assessment.
Now that you have informed the PCBU of what actions are required to manage the scene and you have planned your investigative processes, it is time to investigate why this happened.A Workplace Health and Safety Incident Notification Form has been completed and will assist you with your investigation.In addition, you have obtained the following information from site:

A Worker was installing tiles he fell to the ground, landing upon a stack of timber. He has sustained severe back injuries and received internal injuries.

at 8 Pullen Avenue Craig was on the second story when he has fallen off the roof. The worker was not using a harness.

Craig Turner Injured Worker The following statement was collected from the injured worker the day after the incident. Craig was asked to detail his actions from the beginning of the shift. I was due to start work at 7am, but I had lots to do, so I started at 6am. John (Site Supervisor) wasn't in the site office when I started, but it was unlocked, so I signed in on my time sheet and began work. I wasn't going to wait for him as I'd worked overtime for the last 6 days in a row. I knew what I had to do that day anyway, and it wasn't anything out of the ordinary from my usual job. My plan for the day was to finish installing the tiles on the roof for the south site of the building. I was moving the tiles I needed from stores to up to the roof. It took me several trips to move all the material I needed for the job. Bruno was helping me move some of the tiles, and acted as my spotter when I was climbing up onto the roof. I had all the tiles I needed for the job stacked up on the roof. I went to reach for some more tiles, but they were a little too far for me to grab. I went to move over the reach a tile, and my footing slipped. The roof was still wet from the rain the night prior. I was pretty close to the edge, and I slid off the roof onto a pile of timber that was in a stack next to the building. I was wearing my hard hat, gloves and safety boots.
