Emergency: Fall risk assessment must be completed and documented within 3 days of admission. If at risk, will be assisted with elimination and mobility needs. and other patient-specic interventions will be initiated. Surgical: Fall risk assessment must be completed and documented within 3 days, upon transfer to another level of care. or when a change in condition occurs. Medication Prole Review and Documentation All patient medications must be reviewed for appropriateness by a nurse or doctor within 24 hours of admission. 1v'ital Signs and Documentation All patient vital signs must be taken and documented within 24 hours of admission.
