Develop a flow chart for release of information for a patient whose health care proxy is requesting the information (you can assume the patient lacks capacity). Reflect back on your Medlegal class. We need to make sure there is valid authorization the health care proxy did not use New York State form 960, and the health care proxy, process for picking up in person and/or mailing. Due the change in the HIPAA guidelines the facility has determined that they will charge for labor, material/supplies and postage. For this assignment the facility rate is based on labor and supplies which they have determined their costs on to be 34 cents per page and priority mail/certified returned receipt at $19.75 per request if not picking up in person.

For your flow chart, you must clearly identify the beginning and the end of the process. You must have at least three decision points. It is not necessary for you to list all items that make a valid authorization within your procedure as that could make the process very lengthy.
