Material that pertains to each patient should be filed chronologically within that patients folder with the most current medical documentation on top. Prepare a chart note and new folder whenever a new patient arrives for an appointment. Patient folders are labeled by using either the patients namelast name, then first name (i.e., SHERMAN FLORENCE)or by using the patients account/chart number. Prepare the chart note in this way: Center the words CHART NOTE on the first line. Triple-space; then key information about the patient as shown in the following example. Save the document on your external storage device by the patients last name, followed by the first initial.
