For your assignment the week I want you look back at a previousassignment that we have created. You will be creating an additionalpage for one of your assignments. If you do not make it part of aprior website then make it a stand alone page with a theme. Thiswill be a questionnaire based around that website's theme. If its asports theme, the questions should be based around sports (Asidefrom the typical, first name, last name, etc.) Send the entirewebsite again (including the other html pages) in a zip folder.

Requirements:

• Must have only 1
• Must use the 3 attributes with the proper values in the formtag
  • The form should mail to your Lindenwood email address
• Must have at least 15 different questions / inputs
  • You must use the proper input type for the question.
  • You must use at least 5 different types of inputs (check boxes,drop downs, file uploads, text input, etc)
  • Must have a reset and submit button (that do not count towardsthe 15 questions)
• Each input must have its own unique "name" in the attributesexcept for groups of check boxes and radio buttons.
• Each input must have a value as needed. Exceptions being (forexample) the submit and reset button.
• Must have at least 1 example of a pre-checked or selecteditem
• Must have an appropriate color theme.
• Zip everything up and submit that.
