HELP PLEASE!!!

A true story ... Each month I do an on-line order and did so on Monday. The next day (Tuesday) the manufacturer e-mailed me that there was a new website to use for ordering. Signing up for the website required a new ID (name instead of e-mail address) and a new password. Further, the new website prompted me to place my monthly order because a "Place Order" button was highlighted. However, when going to the "Order History" page, my Monday order was correctly shown as well as my previous order history indicating that my orders had been properly imported from the old website. There also was a button to "Register" for a delivery alert and the text associated with that button stated, "Please note your account number; it is required for registration," and conveniently the text also showed my account number. Clicking the Register" button took me to a page that requested me to input "a single or several Location IDs." Obviously the developers had a few oops (to be polite). Considering what I covered in lecture one, in one sentence state the root (underlying) cause of those oops. A true story ... Each month I do an on-line order and did so on Monday.
