I am doing a question relating to a feedbacK form to be filled in by a colleague who is in my work team.

I am assuming the position of Personal Trainer

The question are

1 Briefly describe your working relationship with the Learner

2 Briefly describe the Learners attitude and competencies at work

a Do they do their work according to industry standards?

b Do they consistently demonstrate a willing and positive attitude towards work?

3 What is one thing tht the Learner can improve on at work?

a Suggest ways in which the Learner can improve on this

4 Other comments

I have several more of these to do so would like to see how you approach it, assuming you are that colleague giving feedback on me..... be kind
