How can I respond to "We use a flow chart to track our patients INR levels. The doctors use this flow chart to make changes to a patient's medication to keep their blood from being too thick or too thin. If a patient's INR level is put in the flow chart wrong it could cause the doctor to change the patient's medication to the wrong dosage and could cause the patient to produce a blood clot or if the patient was to get a cut the patient could bleed to death because their blood would not clot. The only people who are allowed to access the flow charts for patients are our phlebotomist who perform the test and the doctors who prescribe the patient's medications. In our company the phlebotomist usually sends the doctor a message about the patient's results and the doctors do not even look at the chart unless they need to see past results to help see what the patient's dosages have been.
