How do we manage our users?

Regardless of the operating system we use we need to manage users. From a business setting the amount of users that administrators need to support continues to grow. While good for us, it can make things challenging to manage new users, existing ones, terminated ones, etc. So how do you suggest you keep track of all these users? I am not talking about Active Directory or anything like that. I am talking from an organization standpoint.
