Develop Schedule Appointment with your Instructor PHP web site.

Your Web site should include the following features:

1.Student should log in to the system.

2.System should display when instructor is available (2 weeks from the current date).

3.To make an appointment with instructor student should provide the following information:

a.Student name

b.Student ID

c.Select available date

d.Select available time slot

e.Select class number

f.Provide a reason for a meeting

4. Student should be able to cancel his/her appointment.

(Note: system should work only for 1 instructor).
