I need an actual flow chart for permanent filing in HIM!

Current Process: Patient record is created. Patient information is gathered and entered into the system. Patient records are organized alphabetically. Records are filed in the appropriate location. Records are retrieved as needed. Records are returned to the filing location. Records are updated as needed. Records are purged when necessary. New process steps: In the new process, we will use technology to automate and streamline many of the manual steps.

The new process will include the following changes: Patient information will be gathered through electronic forms and patient portals. Records will be organized using an electronic indexing system. Records will be retrieved and updated electronically. Records will be purged automatically based on predetermined criteria. The filing location will be digitized to eliminate the need for physical movement of records.

Bottlenecks could include: Gathering and entering patient information manually. Organizing records alphabetically. Locating and retrieving records from their physical location.
