I need help getting this to normalized E-R diagram that uses the Crows Foot notation

UNIVERSITY MEDICAL CENTERS (UMC) The Company Dr. Joanne Rover is the owner and CEO of UMC, a chain of four medical centers located in Charlotte, North Carolina. The centers are for people without a regular family physician that need occasiona medical care. The centers fill a middle ground between traditional family physician practices and hospital emergency rooms. Most of the patients treated at UMC make advance appointments to be seen at a center. Walk-in patients are also accepted if there are available appointment slots. The centers are open fr om 9 a.m. to 7 p.m., Monday through Friday, and are closed on weekends and holidays Business Analysis Each of the four UMC centers has a unique Center ID number (1, 2, 3, or 4) assigned to it. All centers also have a name e.g., Foxtrot UMC), address, and phone numbe r. Each of the centers employs a number of personnel. A pharmacist license number example would be PH00178. Nurse and pharmacist licenses must be renewed every two years and UMC keeps track of the expiration dates in order to make sure that none of the nurses or pharmacists neglect to renew their license on time. Each UMC center is managed by one administrative employee In addition to these UMC employees, the company also requires the services of contract physicians. These doctors are not considered employees of UMC, but rather are paid as independent contractors to provide medical service at the four UMC centers.
